* Notice of Salary Adjustment pursuant to Sec. 26 of RA 4670

*  Certification of last day of service and leave of absence w/ or w/o pay

*  Complete Service Record

*  Original Copy of Ombudsman Clearance

*  School Clearance

*  Division Clearance

* Provident Clearance

* Original copy of Prosecutor Clearance.

*  Latest Sworn Assets and Liabilities.

* GSIS Policy Contract (if any)

* Photocopy of latest payslip
